PATIENT REGISTRATION FORM
Please complete the Patient Registration Form before your appointment date to reduce the amount of time in the office.
Preparing for your Visit:
Please remember to bring the following:
- Your insurance cards
- A list of current medications
- Your glasses; prescription sunglasses; or if you are a contact lens wearer, a box or packet of contacts for each eye (of your current prescription)
- If your insurance requires a referral, please obtain this from your primary care physician’s office prior to your visit.
- Payment for any portion of the exam not covered by your insurance and co-pays are to be paid at the time of the exam.
